One of the most difficult decisions a person can make is to place an elderly parent or loved one in a Nursing Home. Cabrini offers an excellent alternative. Our Long Term Home Health Care Program (LTHHCP).

This "nursing home without walls" allows families to stay together in the comfort of their own home, yet still provides the range of health care services required by the elderly or the chronically ill.

Each patient is assigned a Case Manager, a public health nurse, who develops, oversees and monitors an individualized plan of care in conjunction with the patient's personal physician.

Residents of Manhattan, Brooklyn or Queens who are chronically ill or disabled, under a doctor's care and in need of preventative, restorative or supportive nursing services are eligible for LTHHCP. In addition, the cost of services must not be expected to exceed 75% of the cost of comparable care in an institutional setting.
