The Cabrini Alzheimer's Care program offers compassion and service with the goal of maintaining quality of life at home and minimizing hospitalizations and emergency room visits. The program includes creative arts services to patients with Alzheimer's disease, informational, support, and respite to family caregivers. The program offers home visits from a nurse practitioner that provides health maintenance, and treatment of acute and chronic conditions.
The volunteer staff assists the patients and their caregivers. These volunteers are carefully screened to ensure a compassionate and caring environment and are drawn from a variety of religious traditions and ethnic backgrounds. They are trained to engage patients in interactive activities such as creative arts, music, and storytelling. These activities are provided for patients at home and in the hospital in order to ease the transition to return home.
The Cabrini Alzheimer's Care program is currently funded by the Fan Fox and Leslie R. Samuels Foundation. The Robert Wood Johnson Foundation Faith in Action, the United Hospital Fund and the George Link, Jr. Charitable Trust has previously funded the program. The Emil & Herzfeld Foundation has awarded the program several grants to provide music therapy to patients.
